The demand is real, but the mistake usually isn't yours
Automated processing gives you credit for exactly what Form 26AS shows, nothing more, even if your Form 16 or payslip shows a different figure. When a deductor quotes the wrong PAN, uses the wrong assessment year, or simply files their quarterly TDS return late, the credit doesn't appear at your end, and the resulting demand looks like your error even though the tax was genuinely deducted and deposited on your behalf.
What people get wrong about this
Filing a Section 154 rectification immediately, before the deductor has actually corrected anything. The rectification can only reflect what's in the record, if the record is still wrong, there's nothing for it to fix yet.
Assuming this is something only the tax department can resolve. In most cases it starts with the deductor filing a correction statement, the department's role is downstream of that, not upstream.
